Requirements

• one letter should be sent by your current Program Director, and three additional letters of reference should be sent by supervisory staff or faculty members with whom you have worked closely (preferably retinal faculty)
• personal statement, stating why you wish to pursue fellowship training in vitreoretinal diseases and your long term career plans
• a copy of your USMLE exam scores
• written verification of any post-graduate clinical training obtained in US hospitals
• medical school transcripts
